welcome dr ibrahim Try to make a differential diagnosis of this appearance on CXR more than one diagnosis. Try to ask your self first what I see. Then make a list of possible doagnoses starting from most common.
Thank you doctor, at 1st i expected that it is an x ray of miliary TB,Bronchopneumonia,Pneumoconiosis. But i thought again after your words i found that there are a large difference ()them as In miliary Tb for example show dots not patches as here (this is what i think) to confirm it please tell me the right ? look here
ALSASLAM ALIKUM DR IBRAHIM, You are welcome for any question. I will guide you in this CXR. Golden rule you are differentiating the radiologic sign, not diagnosing the case. As in ECG, you are treating patient not the ECG. So, all what you mentioned is OK. Military TB is like what you are doing while throwing and distributing the seeds over the chest. the undelying lung tissue is not as in this CXR. Go online and search for CXR MILITARY TB. You will find the difference. ALl the best

BOTH lung fields show evidence of wide spread air spaces( multiple patches scattered and air bronchogram are not well defined).
differential diagnosis:
diffuse histoplasmosis
pneuocysstis carinii
pulmonary hemorrage(hemosiderosis)
alveolar cell carcinoma
lymphoma,....
Thank you doctor,
